*Masters/Doctoral level education in mental health or medical field, and proof of active/good standing provider License or License eligibility is required. **Bachelors level education is required. (relevant work experience can be submitted for review/consideration.) ***Pastoral endorsement and recommendation, and proof of ministry experience is required. Relevant education in theology/divinity is preferred.
